Adaptation, acceptability and feasibility of Problem Management Plus (PM+) intervention to promote the mental health of young people living with HIV in Kenya: formative mixed-methods research

Background
Problem Management Plus (PM+) is a psychological intervention that seeks to address common mental disorders among individuals exposed to adversity. Thus far, the potential for delivering PM+ by mobile phones has not been evaluated.
AimsTo adapt PM+ for telephone delivery (ten weekly sessions of about 45 min each) and preliminarily evaluate its acceptability and feasibility with young people living with HIV (YLWH) in coastal Kenya.
MethodThis was a mixed-method formative research. Qualitative data collection included consultations with stakeholders, conducting key informant interviews with HIV care providers and focus group discussions with potential end-users, i.e. YLWH. Moreover, brief exit interviews with recipients of the adapted PM+ were conducted. Quantitative acceptability and feasibility indicators and outcome measures were tracked/assessed during PM+ preliminary implementation involving 70 YLWH.
ResultsFrom the qualitative inquiries, the adapted PM+ emerged as contextually appropriate, acceptable and feasible for mobile phone delivery, despite some concerns around missing nonverbal cues and poor network connectivity. High recruitment (85%) and fair programme retention (69%) were observed. Intervention sessions over the telephone lasted 46 min on average (range 42–55 min). Preliminary feasibility data indicated that the adapted PM+ has the potential of reducing common mental disorders among YLWH from the Kenyan coast.
ConclusionsPM+ is acceptable and can feasibly be delivered via mobile phone to YLWH in coastal Kenya. This study sets the stage for a future fully powered, randomised controlled trial assessing the efficacy of the adapted PM+ in this or a similar setting.

Productivity of dry bean is constrained by the competition with weeds for scarce nutrients and water in eastern Africa. Trials were conducted at Cornell University in 1996 and in central Uganda during the two seasons of 1997 to test the hypothesis that bean crop nutrition can be improved while increasing the relative competitiveness of bean with annual weed species. Soil levels of available N, P, and K were varied in the main plots. Subplots consisted of bean and two weed species in pure stands and bean mixed with each of the weed species. The weed species were black nightshade and smallflower galinsoga at Cornell and smallflower galinsoga and hairy beggarticks in Uganda. Bean yield was the most suppressed by hairy beggarticks with a mean reduction of 48%. Bean nutrient uptake and growth decreased relative to the weed nutrient uptake and growth when N and P were applied, but the relative competitiveness of bean increased with K application. The K effect on bean yield was greater than the P effect in two out of three trials. Alternative practices for the supply of N and P need to be evaluated for increasing bean yields while reducing the relative benefit to weeds.

X-ray Reflectivity Study of Gold Nanoparticles in a Polymer Matrix

We have examined the structural evolution of a thin gold layer sandwiched between two polymer films using specular X-ray reflectivity. We used a depth-profiling model consisting of three independent layers, upper and bottom PS layers and an intermediate gold layer. The results show that a well-defined as-deposited Au layer of about 62 ± 3 Å breaks within 10 min of annealing at 120 °C. During the breakage of the Au film, the bottom low-Mw PS flows to fill into the void between the Au particles, causing the reduction of the bottom PS film, while the top polymer layer remains relatively constant.

The effect of silica nano-particle filler on poly(ethylene-co-hexene)/xylene gels has been investigated using mainly small angle neutron scattering and rheological measurements. Both non-filled and filled polymer gels show same characteristics in the structure factor and mechanical properties during melting and gelation. These observations suggest that particle interaction is screened by the absorbed polymers and the effect of the silica filler is mainly softening the gelation transition by introducing defects to the crosslinking microcrystals.

The influence of shear on viscoelastic polymer-clay solutions was investigated by means of small-angle neutron scattering (SANS) under shear. SANS measured the shearinduced orientation of polymer and platelets. With increasing shear rate an anisotropic scattering pattern developed. At higher shear rates, the scattering anisotropy increases due to the increased orientation of the clay platelets in the shear field. Cessation of shear leads to fast recovery demonstrating the system to be highly elastic. As a result of drying, these solutions produce translucent nanocomposite films with a microporous membrane character. Depending on the preparation and degree of polymer-clay film dispersion, it is possible to modify the morphology and elastic properties of nanocomposite materials. Atomic Force Microscopy (AFM) reveals the network character and the development of morphology as a function of polymer concentration. Preliminary SANS experiments on the films will be correlated to morphologies obtained from AFM.

The phase behavior of polymer blends and solutions can be changed dramatically by a flow field using a variety of flow geometries. Unlike simple binary fluids which require extremely high shear rates to produce only small shifts in the phase boundary, polymer phase behavior may be influenced by as much as 10 degrees with the application of much lower shear rates. However, there is a large body of conflicting data concerning the nature of these shear effects in polymers.
Here we report on the effects of shear on the phase behavior of polymer blends by small angle neutron scattering (SANS). Experiments were conducted using a specially constructed, concentric cylinder apparatus for in situ studies of concentrated polymer solutions and melts. Two separate systems will be discussed: 1) a blend of polystyrene and polybutadiene. 2) a blend of polystyrene and poly(vinylmethylether). Both systems exhibit shifts in the phase behavior which indicate shear induced mixing in agreement with previous results obtained by other techniques. These results will be interpreted within the context of existing theories of shear induced phase behavior.
Microstructure and Isotopic Labeling Effects on the Miscibility of Polybutadiene Blends Studied by the Small-Angle Neutron Scattering Technique

Deuterated polybutadiene and protonated polybutadiene (PBD/PBH) blends with various microstructures have been studied by the small-angle neutron scattering experiments. Correlation length, ξ, zero-wavenumber structure factor, S(q=o), and interaction parameter, Xblend have been obtained. All PBD/PBH blends exhibit UCST behavior. With the use of random copolymer theory, the interaction parameter, Xblend, has been successfully separated into X1, X2 and X3 which are interaction parameters between the same isotope labeled 1,2-unit and 1,4-unit, opposite isotope labeled 1,2-unit and 1,4- unit, and opposite isotope labeled 1,2-unit with 1,2-unit or 1,4-unit with 1,4-unit, respectively.
